To apply for the JumpStart program, please fill out the APPLICATION, BUDGET worksheet and RELEASE of INFORMATION. forms and submit by regular mail (Attn: Heidi; Community Action Duluth, 19 N. 21st Ave. W, Duluth, MN, 55806), by fax (218.726.1612) or in person at Community Action Duluth.

Any verifications requested (copy of driver's license, pay stubs,etc.) will need to be mailed to or dropped off at Community Action Duluth offices.
