Jump Start Application Materials

 

To apply for the Jump Start Program, please fill out the APPLICATION, BUDGET worksheet and RELEASE of INFORMATION forms and submit by regular mail (Attn: Heidi), by fax (218-726-1612) or in person at Community Action Duluth.

Any verifications requested (copy of driver's license, pay stubs...etc.) will need to be mailed to or dropped off at Community Action Duluth offices.
